Subject: Information regarding 1095C on-line release
91³ÉÈËÊÓƵ Employee:
1095-C’s are is now available to 91³ÉÈËÊÓƵ employees who have given prior consent to receive their 1095-C in PAWS. Every employee of an applicable large employer (such as 91³ÉÈËÊÓƵ) who is eligible for insurance coverage should receive a 1095-C.
If you choose to receive your 1095-C statement electronically, a paper 1095-C will not be printed and mailed.
You may still provide a consent to receive access to your 1095-C now, as well as, prior year forms. This is a one-time consent and will not be necessary to repeat each year. You may revoke your consent at any time by contacting the Payroll Office.
To activate your 1095-C consent please follow these directions:
• Log-in to PAWS: /pawslogin/
• Select the EMPLOYEE tab
• Select TAX FORMS
• Select 1095-C Employer-Provided Health Insurance Offer and Coverage Statement
• Select 2020, then click DISPLAY
• A pop-up message will appear indicating you will need to provide CONSENT. Click OK
• Under MY CHOICE, check the box that indicates you consent to receive your 1095-C electronically.
*Click SUBMIT. (After clicking Submit, your consent will be complete.)
If you did not consent to receive your electronic 1095-C prior to March 2, 2021, you should expect to receive the form via US Mail. Paper forms were mailed to the last known address on file. If you have had an address change during 2020, please contact the Human Resources Office to update your address. If the form is returned to HR and a new address is on file, we will forward to the updated address.
